Every woman going through perimenopause will have heard about hot flushes. Many will have heard about brain fog, disrupted sleep, and mood changes. Far fewer will have heard that the same hormonal upheaval driving all of those symptoms is also quietly altering their cardiovascular risk — often in ways that won't show up on a blood test until years later.
That gap in the conversation is something I think about a great deal. Because what happens to a woman's heart in midlife is not separate from what happens to her hormones. They are the same story.
Oestrogen: your cardiovascular ally
For most of a woman's reproductive life, oestrogen is doing a remarkable amount of work in the cardiovascular system. It keeps blood vessels flexible and responsive. It supports HDL production. It keeps LDL at bay by enhancing the liver's ability to clear it. It reduces inflammation in arterial walls. It has a mild anticoagulant effect. It even plays a role in maintaining healthy blood pressure.
When oestrogen begins to decline — not all at once, but in the jagged, unpredictable way it does during perimenopause — all of these protective effects begin to diminish.
What happens in the blood
The hormonal changes of perimenopause produce a distinctive shift in the lipid profile. LDL cholesterol tends to rise. HDL may fall or become less functional. Triglycerides often increase, particularly in women who have any degree of insulin resistance. The ratio of LDL to HDL — one of the most meaningful markers of cardiovascular risk — often worsens.
These changes do not happen overnight. They tend to unfold gradually over the five to ten years of perimenopause, which means they can be mistaken for gradual dietary deterioration or simple ageing. Many women who seek help after a concerning cholesterol result are told to eat less saturated fat and come back in three months. That advice, while not wrong, is addressing the symptom rather than understanding the mechanism.
Inflammation, insulin resistance, and the missing piece
Declining oestrogen also contributes to increasing central adiposity — weight gain around the abdomen, which drives insulin resistance. Insulin resistance, in turn, drives triglyceride production, suppresses HDL, and produces a particular kind of small, dense LDL particle that is especially prone to oxidation and arterial damage.
This is the aspect of women's cardiovascular risk that is most underappreciated. It is not a simple cholesterol problem. It is a metabolic and inflammatory problem driven by hormonal change — and it requires a response that addresses all three dimensions.
The protective role of lifestyle
The good news — and there is genuine good news here — is that the lifestyle interventions with the strongest cardiovascular evidence align well with what the body needs during perimenopause more broadly.
Movement is the single most powerful tool available. Aerobic exercise raises HDL, reduces triglycerides, improves insulin sensitivity, and directly reduces cardiovascular mortality risk independent of changes in body weight. Resistance training adds to this by improving body composition and glucose metabolism. The evidence for exercise in cardiovascular risk reduction is, if anything, understated.
Dietary pattern matters more than any single food or nutrient. The Mediterranean dietary pattern — high in olive oil, fish, legumes, vegetables, fruits, and moderate in whole grains — has the strongest evidence base for cardiovascular risk reduction in women. It works partly through lipid effects, partly through anti-inflammatory mechanisms, and partly through improved insulin sensitivity.
Sleep and stress management are not optional extras. Chronic poor sleep drives cortisol dysregulation, which drives insulin resistance, which drives the lipid and inflammatory changes described above. The woman who is sleeping badly through night sweats and then managing elevated stress at work is in a very different cardiovascular risk position to her numbers alone might suggest.
What you should ask your GP
If you are in perimenopause and you have not had a full lipid panel in the last two years, ask for one. And when you get the results, ask about more than just the total cholesterol number. Ask about:
- Your LDL and HDL individually, and the ratio between them
- Your triglycerides, particularly if you carry any central weight
- Your fasting glucose or HbA1c, to check for early insulin resistance
- Your blood pressure trend over time
These numbers, together, give you a meaningful picture of where you are — and enough information to take genuinely useful action.
